The health watchdog says weaknesses in governance and management of gynaecology services at Letterkenny University Hospital are continuing to put women at risk.
HIQA says the finding's part of its follow-up review to an independent investigation of complaints in recent years, including delayed cancer diagnosis.
In its findings, the watchdog says weaknesses in governance at the hospital and of oversight at the Saolta Group remain despite 'significant resources' being made available.
