The review found levels of distrust, disrespect and blame between government departments, agencies and non government organisations.
It found examples of fragmentation across and within structures.
According to a report by Safe Ireland and NUI Galway, the average cost of the crime on a woman during her lifetime is more than 113,000 euro, which includes health and legal bills, while also some are left homeless as a result of domestic violence.

The Department of Justice says it will now lead a new Government national strategy to tackle domestic and sexual violence.
Researcher Dr. Carol Ford of NUI Galway says time out of work due to injury, fear or stress is the biggest contributor.
